How can I save the information with Adobe Reader?
Fiachra Forms has created this PDF form with the "local save" capability enabled. Once the form is completed, simply choose "save as" and name the claim. Each separate claim can be opened and edited later if needed. Requires the current version of Adobe Reader.
How can I print the Form* and Text?
Open the file CMS-1500.pdf and fill in the patient information. In your print settings choose "Best Color" or "OCR Red" to produce the Red "dropout" ink for the form, which is invisible to image processing technology. The patient disclaimer is also included for two-sided printing.
How can I print just the text into my own CMS-1500 paper claim?
Place a CMS-1500 paper claim into the printer. Open the file CMS_1500_for_preprinted_forms.pdf and fill in the patient information. Only the fill fields will print in Black Courier 10-point font type.
How do I adjust the print settings of the form?
The PDF itself cannot be adjusted, however we have made adjustments to the template for you. Please try out our adjusted versions prior to purchase.

A licensed user may open, use, access, display and copy the CMS-1500 PDF on up to two (2) computers. This license may not be shared, transferred or used by others.
